Background Check & Safe Environment: Info & Forms

If you have been told you need a background check completed in connection with your activities connected with or through the Church, you will need to complete the following steps:

  1. Print, complete and sign the Volunteer Application.
  2. Print, complete and sign the Background Check Consent Form.
  3. Print and read the Protection of Children and Vulnerable Individuals Policy. Complete and sign the last page of this policy.
  4. Print and read the Code of Conduct. Complete and sign the last page of this policy.
  5. Print and read the Ministerial Standards for Educators and Education Volunteers. Complete and sign the last page of this policy.
  6. Print and read the Ministerial Standards for Persons Dealing with Vulnerable Individuals. Complete and sign the last page of this policy.
  7. View the Safe Environment Video.
  8. Print, complete and sign the Safe Environment Adult Compliance Form.
